Add email forms to your Yahoo! GeoCities site. Create surveys, collect important information, and have the results sent the alternate email address you have listed in your Yahoo! Account Information.
You have a few options to install forms to a page.

Copy & Paste the HTML code for Sample Email Form 1 or Sample Email Form 2 to your page. You may use the forms as they are or edit them to suit your needs.
If you are familiar with HTML forms and/or already have a form created you can add functionality to it by following the steps below.
